Triage and Case Management Assistant - Job description


Overview

We have an exciting vacancy for an experienced triage and case management worker to join our team (0.6/3 days a week) contract, working days are Wednesday to Friday.


Based in beautiful surroundings near a spa resort in Liphook, this is a fantastic environment to work in, among a dedicated and knowledgeable team of professionals. About the Charity

Sporting Chance has a vision to be recognised as the leading provider of Mental Health Services, including addiction, for the world of elite and professional sports. It is a registered charity, and has been established for 25 years. We aim to provide access and pathways to professional, safe and confidential mental health services for those who currently participate in elite and professional sport or have previously done so and for those who are involved in the framework that enables elite and professional sport to function such as coaching, medical or officiating staff.


About the role

We require a part-time (0.6/3 days per week, Wednesday-Friday) triage and case management worker to assist in delivering a high standard of triage support within our service. The role is located at our treatment and triage facility in Liphook, Hampshire.


The primary purpose of this job is to receive, manage and signpost those seeking support and manage the client journey through our national telephone helpline service offered to the world of professional sport.


The successful applicant will join a small, dedicated and professional team who are focused on helping those in need.


It will be essential that applicants are able to demonstrate:


  • Experience of a client facing, triage or key working role in the field of mental health or public support.

  • Experience of initial contact screening and information gathering through telephone and email communication.

  • Experience of working with substance misuse, addiction and abstinence-based recovery.

  • An ability to prioritise and manage a busy case load and level of enquiries.

  • An ability to interpret a variety of emotional / mental health presentations and signpost to relevant pathways of support.

  • An excellent telephone manner with an ability to communicate information to both clients and third-party agencies.

  • Good professional boundaries whilst showing a welcoming level of empathy to clients.

  • A good level of I.T. skills together with an ability to manage data through spreadsheets and database software.

  • Good administration skills and able to record sensitive information in an appropriate and compliant manner (including the GDPR).

  • An experience of working with a high level of confidentiality whilst respecting national safeguarding expectations.


Employment package

The successful candidate will have the option to join the Sporting Chance Group Personal Pension and will be enrolled on to our Health Cashback Plan run by Simply Health. Holiday entitlement is 15 days per annum (0.6 contract) plus public holidays.
